It is with a heavy heart that we announce the passing of our loving father and husband Jerry Milton Chase.

Jerry passed peacefully at home surrounded by his family and loving wife. Jerry is survived by daughter Tammy Farmer, his granddaughters Kady and Amanda Farmer, his twins Steven Chase, his wife Candace Chase and their children Adele, Adam and David, Stephaine Chase and her husband Johnny Farfanick and their son’s Damian and Dawson Farfanick, and his loving wife Anita Fox.

Jerry will be remembered as a fighter until the very end and now rests in peace with his first wife Adele Chase.

Until we meet again Dad, we will never stop loving you.
