Passed away peacefully, on Monday, January 28, 2019, at the Credit Valley Hospital, Mississauga, at the age of 42. Beloved husband of Ewa. Cherished daddy of Sophie. Survived by his parents Wanda and Krzysztof. Loving brother of Anna (Wojciech). Jack will be lovingly remembered by his extended family and friends.
